Referrals
Finding the Right Placement.
At the Right Time.
We work in partnership with local authorities and statutory professionals to assess every referral individually.
Our aim is to ensure every placement is safe, suitable and able to meet the needs of each young person.
Referrals
Every Placement Starts with Careful Assessment
Finding the right placement is about much more than availability.
We work closely with referring professionals to understand each young person’s needs, strengths, risks, and aspirations before making any placement decisions.
Every referral is assessed individually to ensure:
- Needs can be safely met
- Risks can be appropriately managed
- The placement is suitable for both the individual and the service
Admissions & Placement Criteria
All referrals are considered individually to ensure placements are safe, appropriate and able to meet the needs of each young person.
Referrals are typically received from:
- Local Authorities
- Social Workers
- Leaving Care Teams
- Other Statutory Professionals
Who We Can Support
We consider referrals for young people and young adults aged 16–25 who may:
- Be looked after children or care leavers
- Be transitioning from foster care, residential care or secure settings
- Have emotional, behavioural or mental health needs
- Be at risk of homelessness or placement breakdown
Matching & Suitability
Creating Stable Placements
Successful placements begin with successful matching.
Before confirming a placement we carefully consider:
- Individual support needs
- Risk assessments
- Compatibility with existing residents
- Staffing arrangements
- Accommodation type
This careful matching process helps create stable placements that support positive relationships, greater independence and successful outcomes.
Once a placement has been agreed, we work closely with referring professionals to plan a smooth admission. Young people are supported to settle into their accommodation, understand expectations, and begin developing their personalised support plan from day one.
